1. In the general question it should be separated in two category; one for communication tools and services (something to do with the computer, internet and phone lines services) and the second one for management communication system (it has to do with the exchange of information from the top management down to staffs and students, and vice versa). With these separation we will be able to determine whether the weakness is from the hardware services or the management bureaucracy in the college system of communication.
2. for the question number 7, in my humble opinion, I think much better if what is ask the respondent on how to improve the communication system of the college in two category also;
a. hardware side (Tools for communication)
b. exchange of information from the COM-FSM management to the stakeholder.
3. for question number 8. I think the question need not to be included anymore for its asking a story to write. edited 15:15, 14 Mar 2012
- okay with it except for the story part. Instead of story, can we ask them to write an example of when/where they encountered/involved good/bad communication/decision making?
Faculty/Staff version:
-there are support staff/part time faculty that are on special contract, can they check more than one option?
- I think there are redundant questions regarding the revised committee structure, eg. 6b and 6e.
*For both versions, how can we aggregate the answers if the general questions are not numbered? **Thanks to the committee that came up with the surveys.

a) COM Website b) Facebook c) Email d) Postings around college e) Others ________________
In this regard we could really specifically zero out areas that the college should focus on.
I have to add though that the number of questions should not be many or at least it would take only 10 minutes for the respondent to answer the questionnaire. In which by the way Monica's suggestion of easier vocabulary should be employed in order that respondents could answer quickly.
